This form is used to enroll authorized engineers in PVFARM’s internal SMS alert system.
To enroll:
- Enter your mobile phone number.
- Check the box confirming: “I agree to receive operational SMS alerts from PVFARM. Message frequency varies. Message & data rates may apply. Reply STOP to opt out. Reply HELP for assistance.”
- Submit the form.
The checkbox is required and not pre-selected.
Consent is recorded upon submission.
